By submitting an intake through HHMeds, you acknowledge that you have read and agree to this consent.

Nature of telehealth

Care coordinated through HHMeds is primarily asynchronous: you submit an intake and a licensed provider reviews it without a live visit, unless the provider or you request one.

Provider–patient relationship

A provider–patient relationship is established when a clinician reviews your intake and issues guidance or a prescription. Clinicians are independent providers in the LocumTele network, licensed in your state.

Expected benefits

  • Access to a licensed clinician without traveling to an office.
  • Convenient submission of your health history at your own pace.
  • Coordinated dispensing and discreet shipping through a licensed U.S. pharmacy.

Potential risks and limitations

  • The information you provide may be insufficient for a safe clinical decision.
  • Communication delays can occur between you and the provider.
  • Asynchronous review does not replace emergency care or in-person examination.
  • Technical failures may interrupt or delay review.
  • Protected health information could be intercepted despite reasonable safeguards.

Emergencies

Telehealth is not for emergencies. If you are having a medical emergency, call 911.

Confidentiality and records

Your information is handled as described in our HIPAA Notice of Privacy Practices and Privacy Policy.

Your rights

  • Withdraw consent at any time by emailing Chris@HHMeds.com.
  • Request a different provider.
  • Seek in-person care at any time.

Acknowledgment

By submitting an intake, you confirm that you have read this consent, that your answers are accurate and complete, and that you understand a prescription is issued only when a licensed provider determines it is clinically appropriate.

A doctor may request a video call if needed.
