Our commitment

HHMeds, the independent providers in the LocumTele network, and Rush Pharmacy protect your protected health information (PHI) in accordance with HIPAA and applicable state law.

Uses and disclosures for treatment, payment, and operations

  • Treatment: your intake is shared with the licensed clinician evaluating you and with Rush Pharmacy to dispense any prescription issued.
  • Payment: limited information is used to process the consultation fee and medication charges.
  • Health care operations: quality review, support, and administration of the service.

Other permitted or required disclosures

  • As required by law.
  • Public health activities.
  • Reporting abuse, neglect, or domestic violence.
  • Health oversight activities.
  • Judicial and administrative proceedings.
  • Limited law-enforcement circumstances.
  • To avert a serious threat to health or safety.

Your rights

  • Access and obtain a copy of your records.
  • Request an amendment to your records.
  • Receive an accounting of certain disclosures.
  • Request restrictions on uses and disclosures.
  • Request confidential communications.
  • File a complaint without retaliation.

How to exercise your rights or file a complaint

Email Chris@HHMeds.com. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

We may change this notice and apply the revised notice to information we already hold. The current version is always posted on this page.
